About Community Aged Care Packages

A Community Aged Care Package is a planned coordinated program of care offered to a person who has complex care needs. The package will assist the person to remain in their own home by providing a range of day-to-day and home maintenance services planned to meet their specific needs.

Who can use the program?

To be eligible for a Community Aged Care Package you must:

• Have expressed a desire to remain living in your own home
• Be eligible for at least a low level of residential care
• Have been approved as suitable by the Aged Care Assessment Service
