About Home and Community Care

Home and Community Care provides a range of practical services to help people to be cared for in their home. The program aims to help people to live at home as long as possible, rather than move into a nursing home or hostel before they are ready.

Who can use the program?

You may be eligible if you are:

• A frail older person
• A younger person with a disability
• A carer

Home and Community Care funded projects that provide the following:

• Centre based respite
• In home respite
• Assistance with daily living skills
• Assistance with daily living tasks
• Recreational and educational opportunities
• Holidays
• Emergency support
• Overnight/Weekend support
• Family/Carer support
• Transport
• Meal provision
