Community Aged Care Package (CACP)

The program is funded by the Australian Department of Health and Ageing and is a higher level of service provision to maintain clients in their own homes.

CACP is a planned co-ordinated program of care offered to a person who has complex care needs. The package assists a person to remain in their own home by providing a range of day-to-day and home maintenance services planned to meet their specific needs.

Eligibility for Services

• You have expressed a desire to remain living in your own home
• Following referral by a general practitioner to the Aged Care Assessment Team you have been assessed and approved as suitable for a Community Aged Care Package
