Is a support program for carers of frail elderly people and people living with dementia.
Carers are people who look after someone who is frail aged, living with dementia, or has a disability, a mental illness, a chronic illness, an intellectual disability, or a physical disability. They can be a family member, friend or neighbour. Caring can be challenging and without appropriate support, carers can be at an increased risk of poor psychological and physical health and wellbeing.
SHARE THE CARE aims to support carers in five key areas: providing person-focused Services; activities to promote the Health and wellbeing of carers; accessing Assistance from agencies and other service providers; offering respite options to encourage Rest and relaxation; and providing Education to raise awareness, build knowledge and strengthen skills.
The Program team is committed to understanding a carer's unique needs and preferences and tailoring a support program to match those needs. The Program is flexible to suit individual requirements and participation is optional.
