Centacare provides programs that are specifically designed for young persons with disability support needs.
Two of these programs include:
- Post School Operations Program (Northside)
- Soubirous Place (Southside)
These programs are explained below:
Post School Operations Program
Centacare Disability Services also supports young persons with disabilities who have finished their schooling through two funded programs.
- The Moving Ahead Program
- Options Plus Package
These programs allow young persons to receive individualized support to make the transition from school to early adulthood and aim to develop their identity as a valued member of their community.
Services on the North and Southside of Brisbane are able to provide support to young adults who receive funding from these two program areas.
Soubirous Place
Soubirous Place targets young persons with disabilities who lack other opportunities for personal development and who live within the geographical boundaries of Sunnybank, Upper Mt Gravatt, Salisbury, Acacia Ridge and Wishart. Persons who receive Moving Ahead funding may also choose to participate in these programs if it suits their needs.
It operates under the Southside Disability Centre.
Soubirous Place is jointly funded by HACC and five local Parishes who are interested in providing options for young people with a disability.

- Communication
- Gym and Swim
- Community Access
- Literacty and Numeracy
- Art Classes
- Drama
